CONS-10 // PROCESS HAZARD ANALYSIS
HAZOP & LOPA Studies
Hazard and operability studies on your P&IDs, run node by node with guidewords to find how deviations can lead to harm, environmental damage, or lost production. Where a scenario needs more than team judgement, Layer of Protection Analysis tests whether the independent protection layers are enough and sets the SIL target for instrumented safety functions.
WHO IT IS FOR
Refineries, gas plants, upstream production facilities, petrochemical and chemical plants, terminals, and EPC contractors: at detailed design, before start-up, after modifications, and at periodic revalidation of existing units.
WHEN YOU NEED IT
P&IDs reaching approved-for-design status, a modification under MOC that changes process conditions, a scheduled HAZOP revalidation, an incident or near miss on the unit, or an insurer, partner, or authority asking for a current process hazard analysis.
WHAT YOU RECEIVE
- Node breakdown, design intent, and deviation matrix agreed before the sessions
- Facilitated HAZOP sessions with an independent chair and a dedicated scribe
- HAZOP worksheets with causes, consequences, safeguards, and recommendations
- LOPA for high-consequence scenarios and SIL targets for safety instrumented functions
- Final report and an action tracker your team can close out and verify
WHAT WE NEED FROM YOU
- Current P&IDs, PFDs, heat and material balance, and cause and effect diagrams
- Operating procedures, alarm and trip settings, and relief valve data
- Incident history and previous HAZOP reports for the unit, where available
- A core team at every session: process, operations, instrumentation, and maintenance
FREQUENTLY ASKED QUESTIONS
How often should a HAZOP be revalidated?
Many operators revalidate every five years, the interval used in OSHA process safety management practice that many companies follow. Your corporate standard or the owner's requirements may set a different interval, and a significant modification should go through HAZOP or another hazard review under MOC.
Do you also carry out SIL verification?
Our studies set the SIL target through LOPA. Checking that a designed safety instrumented function meets its target (SIL verification) needs reliability data for the actual equipment. Where you need it, we coordinate the work with functional safety specialists.
Can the sessions run online?
Partly. Revalidations and smaller studies can run online with shared worksheets. For a new design we recommend sessions in the room, because the discussion is better and the team stays focused.
