HSSE APPS // BEHAVIOUR-BASED SAFETY
Observation Card
Behaviour-based safety observation: observe, intervene, discuss, then record what you saw without naming anyone.
OVERVIEW
What This Application Does
Observation Card is a behaviour-based safety observation programme in digital form. Supervisors and workers observe a task, stop it if someone is at immediate risk, talk with the people involved about what they are doing safely and what puts them at risk, agree on a safer way, and then record the observation on the card. The card counts safe and at-risk behaviours in five categories: personal protective equipment, positions of people, tools and equipment, procedures, and housekeeping. The people observed are never named, because the aim is a conversation and not blame.
Repeated at-risk behaviours are turned into coaching sessions, followed by a new observation to check that the change holds. Trend charts show at-risk behaviour by category, department, shift and day, and a participation board ranks departments by cards submitted against target and by the quality of the discussion recorded. Nusa Safety sets up the categories, targets and coaching flow with the client team as part of a consulting engagement, and the method can be aligned with an observation programme the client already runs.

FUNCTIONS
Built Around Real Workflows
Each function below replaces a step that used to run on paper, spreadsheets, or chat groups.
Observe, intervene, discuss, record
The card follows the conversation in four steps, so recording comes after the talk with the workers.
Five behaviour categories
Personal protective equipment, positions of people, tools and equipment, procedures, and housekeeping, each with detailed items.
Safe and at-risk counts
Safe and at-risk behaviours are counted per item, giving a safe behaviour percentage for every card.
No names, no blame
The people observed are not recorded, only the area, the department and the number of people.
Discussion record
Safe behaviours recognised, at-risk behaviours discussed and the agreement reached with the workers.
Coaching follow-up
Repeated at-risk behaviours become coaching sessions with a coach, a plan and a follow-up observation.
Trend analysis
At-risk behaviour by category, department, shift and day of the week, with a Pareto of the most frequent items.
Participation board
Departments ranked by cards submitted against target, card quality and active observers.
Card quality
Tracks whether cards include a discussion, an agreement and an intervention, not only ticks.
Quick entry on a phone
Counters per item and collapsible categories for recording on the shop floor.
Reports and exports
Monthly participation and trend reports in PDF and Excel.
