PVC safety warning tag, 76 x 143 mm, with a prohibition symbol and 'Do not close valve' on a red panel, and lines for remarks, date and signature. It shows who is responsible for placing the lockout device and who is authorized to remove or change it, communicating lockout status at the point of isolation. The tag can be written with an erasable pen for repeated use, and other wording, designs or a company logo can be custom made.

Using Lockout Tags
A tag warns and identifies; it does not physically stop anyone. Tags are used together with locks, or alone only where a device cannot be locked and the procedure allows it.
- Write the worker's name, the date and the reason on every tag.
- Choose a material that stays legible in the environment: heat, moisture or chemicals.
- Use a standard format and the language the workforce reads; bilingual Indonesian and English tags help on mixed sites.
- Attach tags with a non-reusable tie strong enough not to be removed by accident.
